• To best facilitate scheduling, please submit presentation requests at least six weeks in advance
• We may not be able to accommodate all requests due to staffing availability

Fill out the form and provide the following information:

• Requestor name
• Requestor daytime contact number
• Requestor email
• Group or organization name
• Requested presentation location
• Requested presentation date
• Requested presentation time
• Alternative date(s)
• Number of expected attendees
• Preferred presentation topic(s), such as:
  • Preventing mosquitoes at home
  • Preventing mosquitoes in the workplace
  • Mosquito management techniques
  • The lifecycle and habitats of common mosquitoes
